Questionnaires

To have a Finance community questionnaire completed, please forward a copy of the questionnaire to docs@metropolitanmgmt.com or fax to 866-736-1926.

Please be sure to include the community name, and address of the home for which the form is being completed, in order for efficient processing.

Questionnaires have a cost of $150.00 up to 5 pages. Additional pages are $25.00 up to 8 pages. Any other requests for additional documents may carry a surcharge based on the complexity of the questionnaire.

Credit card and check payments are accepted.

Pay by Credit Card (we do not accept American Express) or Pay by check.

Checks should be made payable to Metropolitan Management Group LLC and mailed to PO Box 1029 Glen Burnie MD 21060.

Once payment is confirmed, our goal for turnaround time for completed questionnaires is 2 Business Days, however, it may take longer due to the length of the questionnaire, questions posed or other items which may require further research.
